Job summary

Oak Street Health is seeking a Center Nurse to join a value-based care team, performing clinical assessments, chronic condition management, wound care, and vaccine administration. You will educate patients and caregivers, collaborate with the care team, and help reduce avoidable hospitalizations.

This full-time position reports to the Practice Manager and involves onsite testing, patient education, and coordination of care transitions to provide high-quality, accessible care to our communities.

Qualifications

• Active, Unrestricted RN licensure in the state of residence.
• BLS or CPR certification required.
• Fluent written and spoken English.
• US work authorization.
• EMR experience and ability to maintain patient confidentiality.

Responsibilities

• Assess patients and triage to ensure appropriate care.
• Perform wound care and vaccine administration.
• Educate patients and caregivers on chronic conditions and preventative care.
• Review medications and conduct nurse-led visits for safety and adherence.
• Coordinate with providers to manage complex care plans and transitions from hospital to clinic.
• Utilize standardized protocols for medication management and renewals.
• Ensure timely documentation and closure of patient visit encounters.
