Job summary

Oak Street Health is seeking a Center Nurse to join our Value-Based Care model, driving quality care through assessments, chronic disease management, and collaborative teamwork. The Center Nurse supports standardized protocols, patient education, and safe medication management while reporting to the Practice Manager.

You will perform wound care, vaccines, triage, and coordination with providers to prevent hospitalizations, ensuring timely care and patient safety across the clinic setting.

Qualifications

• Active, unrestricted RN licensure in the state of residence.
• BLS or CPR certification required.
• Fluent written and spoken English.
• US work authorization.
• Experience using Electronic Medical Records (EMR).
• Ability to maintain patient confidentiality.
• Strong communication and assessment skills, including non-face-to-face interactions.

Responsibilities

• Support patient assessment and triage for walk-in and telephonic visits to ensure appropriate care and avoid unnecessary hospital visits.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and vaccine administration.
• Educate patients and caregivers on chronic conditions and preventative care.
• Review medications during nurse-led visits to ensure safety and adherence.
• Conduct onsite testing (point-of-care labs, EKGs) and coordinate care transitions as needed.
