Qualifications

• Active, unrestricted RN licensure in the applicable state.
• Current BLS/CPR certification.
• Fluent written and spoken English required.
• Authorized to work in the US.
• Experience using Electronic Medical Records (EMR).
• Ability to maintain patient confidentiality and handle sensitive information.
• Strong communication and assessment skills for remote and in-person care.

Responsibilities

• Support patient assessment and triage, for walk-ins and telephonic needs, to ensure appropriate care and avoid unnecessary visits.
• Execute protocol-driven chronic condition management.
• Perform wound care and medication administration, including vaccines.
• Educate patients and caregivers on chronic conditions and preventive care in person, by phone, or in groups.
• Review medications and ensure safety and adherence during nurse-led visits.
• Conduct onsite testing (point-of-care labs, EKGs, specimen collection) and use data to close care gaps.
• Identify and address high-priority lab results to maintain safety and continuity of care.
• Collaborate with providers to manage complex care plans and coordinate transitions from hospital to clinic.
• Follow protocols for medication management, renewals, and prior authorizations.
• Ensure timely clinical communications and close patient visit encounters; delegate to medical assistants and phlebotomists as needed.
