Job summary

Oak Street Health is hiring a Registered Nurse in the United States. The Center Nurse operates within a Value-Based Care model, guiding quality care, chronic condition management and team collaboration to prevent hospitalizations.

You will educate patients and coordinate with providers to support transitions from hospital to clinic. You will perform wound care, vaccines, medication management, and onsite testing while ensuring patient safety and data integrity.

Qualifications

• Active, Unrestricted RN licensure in the state you reside.
• BLS or CPR Certification.
• Fluent written and spoken English.
• US work authorization.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality and process information securely.
• Ability to assess patients without face‑to‑face interaction with strong communication and assessment skills.

Responsibilities

• Support patient assessment and prioritization for triage to ensure appropriate care.
• Execute protocol-driven chronic condition management.
• Provide wound care and vaccine administration.
• Educate patients and caregivers on chronic conditions and preventative care.
• Conduct medication reviews and ensure safety; review labs for continuity of care.
