Oak Street Health, part of CVS Health, is hiring a Center Nurse in New York to join a value-based care team focused on chronic condition management and patient safety. You will lead assessments, perform nursing procedures, educate patients and collaborate with interdisciplinary teams to minimize hospitalizations.

The role reports to the Practice Manager and requires RN licensure in New York, BLS/CPR, and EMR experience.

Qualifications

• Active, Unrestricted Registered Nurse Licensure in the state you reside.
• BLS or CPR Certification.
• Fluent written and spoken English language skills.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality and handle information confidentially.
• Ability to assess patients without face-to-face interaction with strong communication and assessment skills.

Responsibilities

• Support patient assessment and prioritization for walk-in and telephonic triage to ensure appropriate care and avoid unnecessary visits.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and medication administration (including vaccines).
• Educate patients and caregivers on chronic conditions and preventative care in person, by phone, or in groups.
• Review medications within nurse-led visits to ensure safety and adherence.
• Conduct onsite testing (point-of-care labs, EKGs, specimen collection) and use data to close gaps in preventative screenings.
