Job summary

Oak Street Health in New York is seeking a Center Nurse to deliver high-quality care within a Value‑Based Care model. You will perform assessments, chronic condition management, and vaccines, collaborating with the care team to prevent hospitalizations.

This full‑time role reports to the Practice Manager and emphasizes patient education, coordination with physicians, onsite testing, and adherence to safety and quality standards.

Qualifications

• Active, Unrestricted Registered Nurse Licensure in the resident state.
• BLS or CPR Certification.
• Fluent written and spoken English language skills.
• US work authorization.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality and process information confidentially.
• Ability to assess patients without face-to-face interaction, strong communication and assessment skills.

Responsibilities

• Supports patient assessment and prioritization for triage to ensure appropriate care and avoid unnecessary hospital visits.
• Execute protocol-driven chronic condition management.
• Perform nursing procedures including wound care and vaccination administration.
• Provide patient and caregiver education on chronic conditions and preventative care in person, over the phone, or in group settings.
• Conduct medication reviews to ensure safety and adherence.
• Onsite testing and data review to close gaps in preventative screenings and ensure state scope compliance.
• Review abnormal lab results promptly to maintain patient safety.
• Coordinate with providers to manage complex care plans and weekly transitions from hospital to clinic.
• Use standardized protocols for medication management and prior authorizations.
• Ensure timely closure of patient visit encounters and delegated tasks.
