Job summary

Oak Street Health, part of CVS Health, is seeking a Registered Nurse for a full-time Center Nurse role in Jamaica, NY. The Center Nurse operates within a Value-Based Care model, delivering chronic condition management, wound care, vaccines, and patient education while coordinating with a multidisciplinary team.

You will perform onsite testing, conduct medication reviews, and ensure safety and continuity of care, reporting to the Practice Manager.

Qualifications

• Active, Unrestricted RN licensure in the state you reside.
• BLS or CPR certification.
• Fluent written and spoken English.
• US work authorization.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality and process information confidentially.
• Ability to assess patients without face-to-face interaction, strong communication and assessment skills.

Responsibilities

• Support patient assessment and triage to ensure appropriate care and reduce hospital visits.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and vaccines.
• Provide patient and caregiver education regarding chronic conditions and preventative care.
• Conduct medication reviews during nurse-led visits to ensure safety and adherence.
• Conduct onsite testing (point-of-care labs, EKGs, specimens) and identify gaps in preventative care.
• Review and act on high-priority abnormal lab results to maintain safety and continuity of care.
• Coordinate with providers and care team to manage complex care plans and weekly transitions from hospital to clinic.
• Utilize protocols for medication management, renewals and prior authorizations.
• Ensure clinical communications are managed and visits are closed timely.
• Delegate tasks to medical assistants and phlebotomists as needed.
• Performs other related duties as assigned.
• Supports nursing practice and SOP implementation.
• Identifies barriers to optimal care (language, literacy, culture, etc.).
• Creates welcoming environment to meet patient/community needs.
• Delivers exceptional patient experience through service and responsiveness.
• Acts as clinical voice in daily team meetings.
