Qualifications

• Active, Unrestricted RN licensure in the state of residence.
• BLS or CPR certification.
• US work authorization
• Electronic Medical Record (EMR) experience
• Fluent written and spoken English

Responsibilities

• Support patient assessment and triage to ensure appropriate care and reduce unnecessary visits.
• Execute protocol-driven chronic condition management.
• Perform wound care and administer medications including vaccines.
• Educate patients and caregivers on chronic conditions and preventive care.
• Conduct medication reviews for safety and adherence.
• Conduct onsite testing (point-of-care labs, EKGs, specimen collection) and close gaps in screenings.
• Review abnormal lab results promptly to maintain safety and continuity.
• Coordinate with providers to manage complex care plans and weekly transitions from hospital to clinic.
• Utilize standardized protocols for medication management, renewals and prior authorizations.
• Ensure timely documentation and closure of patient encounters; delegate tasks to MAs and phlebotomists.
