Oak Street Health is seeking a Center Nurse to join our clinically oriented team in a value-based care model. You will perform assessments, chronic condition management, and medication administration including vaccines, while collaborating with the care team to reduce hospitalizations.

Qualifications

• Active RN license in the state you reside.
• BLS or CPR certification.
• Fluent written and spoken English.
• US work authorization.
• Experience with Electronic Medical Records (EMR).
• Ability to maintain patient confidentiality.
• Ability to assess patients without face-to-face interaction; strong communication skills.

Responsibilities

• Support patient assessment and triage to ensure appropriate level of care and avoid unnecessary visits.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and vaccines.
• Educate patients and caregivers on chronic conditions and preventative care.
• Conduct medication reviews during nurse-led visits for safety and adherence.
• Perform onsite testing (point-of-care labs, EKGs, specimen collection) and address gaps in screenings.
