Job summary

Care New England Health System in Providence, RI, is seeking a Providence Center Case Manager within the CCBHC program to support clients in reducing psychiatric and substance use symptoms, connecting them to housing, healthcare, and community resources.

You will conduct comprehensive assessments, develop individualized care plans, provide in-person and community-based services, and collaborate with a multidisciplinary team while maintaining compliance with standards.

Qualifications

Associate's degree in Human Services or related field; Bachelor's preferred.
Experience in Human Services field preferred.
Must have valid driver's license, registration, current inspection sticker and insured auto.
Bilingual preferred.

Responsibilities

Coordinate and deliver case management as part of a multidisciplinary team.
Provide services to assigned caseload with in-person contacts in office and community settings.
Conduct comprehensive assessments to identify housing, education, employment, healthcare and community needs.
Develop, present and implement individualized care plans with the care team and clients.
Connect clients to housing, healthcare, education and community resources.
Provide ongoing monitoring and follow-up to ensure needs are met and progress is achieved.
Align case management with clients' mental health treatment plans.
Perform crisis intervention and risk management as needed, escalate when indicated.
Serve as a liaison between clients and community agencies to access services.
Provide education to clients and families on navigating systems.
