Job summary

Catholic Charities of Baltimore is seeking a Case Manager for the Weinberg Housing and Resource Center in Baltimore, MD. The role assesses household needs, coordinates resources based on goals, provides advocacy, information, and referrals to program participants.

The work schedule is Tuesday–Saturday, 9 AM–5 PM. The Case Manager builds individualized service plans, maintains files, reports program data, and collaborates with staff to support housing placement and wellness goals in a

Qualifications

Bachelor's degree in Human Services, Social Work, or a related field.
Two years of progressively responsible experience in case management, social services, crisis intervention, or related area.
Equivalent combination of education, training, certification, and experience.
Knowledge of case management and crisis intervention practices.

Responsibilities

Assess participant and household needs at intake and service closure using the Arizona Self-Sufficiency Matrix.
Develop ISPs collaboratively with participants and monitor progress to completion.
Maintain participant contact and conduct home or community visits as applicable.
Apply Motivational Interviewing and Trauma-Informed Care principles.
Connect participants with eligible benefits and community resources; support applications and referrals.
Advocate for individuals and families with service providers to secure support.
Maintain complete, accurate case files and Service Point records.
Collect, analyze, and report program data; identify trends and risks.
Collaborate with staff and partners to coordinate services.
Participate in required trainings and program events.
Perform other duties as assigned.
