Job summary

Catholic Charities of Baltimore seeks a Case Manager at the Weinberg Housing and Resource Center in Baltimore, MD. The role focuses on assessing needs, developing ISPs, advocating for clients, and coordinating services to help individuals achieve stable housing and well-being.

The position requires a Bachelor’s degree in a human services field and at least two years of related experience, plus strong communication, organizational, and trauma-informed care skills.

Qualifications

• Bachelor’s degree in Human Services, Social Work, or a related field.
• Two years of progressively responsible experience in case management, social services, crisis intervention, or related area.
• Knowledge of case management and crisis intervention practices.
• Equivalent combination of education, training, certification, and experience may be considered.

Responsibilities

• Assess participant needs at intake and closure using standardized tools; guide service planning.
• Develop ISPs with participants and monitor progress toward goals.
• Maintain participant contact and conduct home or community visits.
• Apply Motivational Interviewing and Trauma-Informed Care principles.
• Connect participants with benefits and community resources; assist with referrals.
• Advocate for individuals with providers to secure support.
• Maintain complete case files and reporting requirements.
• Collect and report program data; identify trends and risks.
• Collaborate with staff and partners to coordinate services.
• Attend meetings/trainings and apply learning to improve outcomes.
• Perform other duties as assigned.
