Job summary

Catholic Charities of Baltimore is seeking a Crisis Prevention Coordinator to support clients at the Weinberg Housing and Resource Center in Baltimore. The role focuses on crisis intervention, stabilization, and linkage to behavioral health and housing resources, with a priority on reducing crisis escalations and promoting safety.

The position requires a Bachelor’s degree in a related field and at least two years of relevant experience, with strong communication and collaboration skills to work

Qualifications

• Bachelor’s degree in Human Services, Social Work, or related field.
• Minimum of two years of related professional experience with at-risk populations.
• Experience working with individuals in crisis, including mental health or housing instability.
• Equivalent combination of education and experience may be considered.

Responsibilities

• Build therapeutic relationships with clients and support care planning.
• Promote housing stability by implementing interventions and supports.
• Provide crisis intervention and stabilization with referrals to medical/mental health resources.
• Coordinate with providers to ensure continuity of care and appropriate service utilization.
• Engage clients to reduce emergency service utilization.
• Maintain accurate documentation of client interactions and outcomes.
