Job summary

Catholic Charities of Baltimore is seeking a Case Manager for the Weinberg Housing and Resource Center to support adults facing mental health and housing challenges. The role involves assessment, resource coordination, advocacy, information, and referrals.

Schedule is Tuesday–Saturday, 9 AM–5 PM. Requirements include a Bachelor’s degree in Human Services or related field and at least two years of case management or crisis intervention experience.

Qualifications

• Bachelor’s degree in Human Services, Social Work, or related field.
• Two years of progressively responsible case management or related experience.
• Knowledge of case management and crisis intervention practices.
• Equivalent combination of education, training, and experience may be considered.

Responsibilities

• Assesses participant needs at intake and closure using standardized measures.
• Develops ISPs with measurable goals and timelines.
• Maintains participant contact and conducts home visits as needed.
• Applies Motivational Interviewing and Trauma‑Informed Care principles.
• Connects participants with benefits and community resources; follows up on access.
• Advocates for participants with service providers to secure support.
• Maintains complete case files and HMIS records with accuracy.
• Collects and reports program data; identifies trends and outcomes.
• Collaborates with staff to coordinate services and align with goals.
• Participates in meetings and trainings; stays current on available services.
