Job summary

South Middlesex Opportunity Council, Inc. is seeking a Case Manager in Worcester, MA to provide supportive services for chronically homeless individuals. The role includes initial assessments, ongoing follow-up, and linkage to community resources.

Qualifications

• Bachelor’s Degree in related field. May be substituted with a minimum of 5 years working with the target population.
• Substance abuse experience required.
• Excellent interpersonal and organizational skills.
• Previous experience with group facilitation preferred.
• Must have valid Driver’s License, reliable transportation and meet insurance standards.

Responsibilities

• Ensure adherence to policies and protocols for programs serving chronically homeless individuals, including guidelines for meals, curfews, meeting attendance, client transportation to appointments, etc.
• Develop regular program schedule including activities, meetings, meals, etc.; access external resources as appropriate, including guest speakers.
• Ensure all participants meet HUD requirements for entry into the program and that all required documentation is included in the client file.
• Conduct initial service needs assessment with all clients entering the program and develop Individual Service Plan within three days of entry.
• Work closely with Shelter Staff, Relapse Prevention Managers and Sober Housing staff to identify new participants.
• Maintain daily contact with clients actively involved in the program and establish a regular contact schedule for at least 24 months.
