South Middlesex Opportunity Council, Inc. is hiring a Case Manager for Supportive Services in Framingham, MA. Qualifications

• Bachelor’s Degree in related field; may be substituted with 5 years experience with the target population.
• Substance abuse experience required.
• Excellent interpersonal and organizational skills.
• Previous experience with group facilitation preferred.

Responsibilities

• Assess participants entering the program and develop an Individual Service Plan within three days.
• Coordinate with Shelter Staff, Relapse Prevention Managers and Sober Housing staff to identify new participants.
• Maintain daily contact with clients and establish regular contact for at least 24 months.
• Ensure documentation and HUD requirements are met for client files and reporting.
