Job summary

Community Counseling of Bristol County Incorporated (CCBC) seeks a Social Worker for the Supportive Housing Case Manager role to provide intensive case management to clients recently housed and transitioning out of homelessness. The position requires frequent client transportation and engagement in a variety of settings, including encampments and shelters.

Qualifications

• Bachelor’s degree or 3–5 years of homeless-services experience.
• Experience communicating with diverse populations and collaborating with partners.
• Ability to drive throughout the coverage area and transport clients.
• Proficient in Microsoft Office 365 and learning new systems.

Responsibilities

• Coordinate with client, medical, and mainstream providers to ensure wrap around services.
• Collaborate with housing team to keep clients housed and support permanent housing goals.
• Assist with applications for subsidized housing and benefits to increase income.
• Engage with landlords and providers to establish housing for households in crisis.
• Provide transportation to services and accompany clients as needed.
• Meet with clients in community settings to access services and build rapport and advocacy.
• Opportunities for hybrid work with community-based and office settings.
• Work with collateral agencies to locate housing for homeless clients.
• Complete Needs Assessments and Individualized Action Plans for each client.
