Job summary

Community Counseling of Bristol County Incorporated is seeking a Supportive Housing Case Manager to provide intensive case management for formerly unsheltered individuals who have recently secured housing. You will work with a multidisciplinary team to ensure wrap-around services, document progress, and help clients sustain housing in a supportive community setting.

Qualifications

• Bachelor’s degree in Social Work, Sociology, Psychology, or Human Services or 3–5 years of homelessness experience
• Knowledge of local referral sources and community resources
• Valid driver's license and ability to transport clients
• Strong verbal and written communication, with respectful, compassionate approach
• Proficient with Microsoft Office 365 and basic computer systems

Responsibilities

• Coordinate with clients, medical and community providers to arrange wrap-around services after housing
• Collaborate with housing team to develop supports for Permanent Supportive Housing
• Assist with applications for subsidized housing and benefits
• Engage with landlords and service providers to secure housing for households in crisis
• Provide transportation for clients to services and appointments
• Meet clients in community settings to access services and build rapport
• Occasionally participate in hybrid work arrangements
• Work with collateral agencies to locate housing for the homeless
• Create Needs Assessments and Action Plans for each client
