SMOC is seeking a Case Manager, Supportive Services in Worcester, MA. The role provides case management to episodically homeless individuals, including assessment, referrals, home visits, and group facilitation. Hybrid work options are available with 0-20% remote scheduling.

The position requires a Bachelor’s degree or 5 years’ experience with the target population, substance abuse experience, and strong interpersonal and organizational skills.

Qualifikationen

• Bachelor’s degree required; substitute with 5 years of relevant experience.
• Substance abuse experience required.
• Excellent interpersonal and organizational skills.

Aufgaben

• Ensure adherence to policies and protocols for programs serving chronically homeless individuals.
• Develop regular program schedule including activities, meetings, meals, etc.
• Ensure HUD requirements for entry into the program and documentation in client files.
• Conduct initial service needs assessment for new clients.
• Develop Individual Service Plans within three days of entry.
• Maintain daily contact with program participants.
• Provide crisis intervention (24-hour cell phone coverage).
• Attend engagement team meetings and coordinate with staff.
