Job summary

Qualifications

• Bachelor's degree or equivalent experience in social services or related field.
• Minimum 2 years' case management experience with low-income populations preferred.
• Experience coordinating services and documenting case progress.

• Facilitates case management within program guidelines; develops rapport; creates case plans; tracks progress; coordinates referrals.
• Maintains relationships with community agencies; promotes services; ensures releases of information are on file.
• Creates case plans and connects clients to resources; monitors progress and outcomes.
• Participates in trainings and meetings; supports ad hoc reporting and data entry.
• Uses electronic databases to maintain accurate case records and manage service transactions.
• Performs other duties as assigned.
