Zusammenfassung

Vincentian CARES in St. Petersburg, FL is seeking a Case Manager IV to provide assessment, planning and advocacy for housing and supportive services for very low-income homeless seniors and veterans.

You will manage an assigned caseload and coordinate with community partners to secure permanent housing. The role requires strong knowledge of harm reduction, trauma‑informed care, and data collection using Case Worthy/HSMIS.

Qualifikationen

• Bachelor’s degree or equivalent experience in social services.
• Minimum 5 years of relevant social services experience preferred.
• Strong communication and organizational skills are essential.
• Ability to pass Law Enforcement background screening.
• Valid Florida driver’s license and reliable transportation.
• Flexible schedule including evenings and weekends.
• Demonstrates cultural sensitivity with diverse populations.

Aufgaben

• Provide crisis intervention to address immediate needs of very low-income, homeless seniors.
• Assess strengths, needs, abilities to help develop housing goals.
• Enforce program rules and ensure compliance with government/contract regulations.
• Coordinate with clients and community resources for housing suitability.
• Move clients toward permanent housing and coordinate related services.
• Maintain caseload records and data using Case Worthy and HSMIS.
• Serve as liaison to community agencies and providers.
