St. Vincent de Paul CARES in Saint Petersburg, FL is seeking a Case Manager IV to assess, plan, monitor, and advocate for housing services for very low‑income and homeless individuals and families.

The role focuses on determining eligibility, coordinating resources, and maintaining accurate case records. Qualifications include a Bachelor's degree or 5 years in social services, strong English and computer skills, and willingness to work flexible hours.

Qualifications

• Bachelor’s degree and/or 5 years in social services.
• Experience with homeless populations and veterans preferred.
• Proficiency with data management systems and MS Office.

Responsabilités

• Provide crisis intervention to address immediate needs of the very low-income, chronic and non‑chronic homeless senior population.
• Assess strengths and needs to develop housing goals.
• Coordinate move to permanent housing and link to services.
• Ensure compliance with government and contract regulations.
• Maintain and monitor caseload and data in Case Worthy/HSMIS.
• Liaise with community agencies and providers.
