Job summary

St. Vincent de Paul CARES in San Juan, PR, is seeking a Case Manager III to provide assessment, planning, linkage and advocacy for veterans and their families in the Supportive Services for Veteran Families program.

You will manage a caseload, develop Housing Stability Plans, coordinate housing services, monitor progress, and ensure compliance with government and contract regulations; strong English, computer skills, and teamwork are required.

Qualifications

• Bachelor’s degree in social work or related field required.
• Minimum two years’ experience serving homeless or at-risk families/individuals in crisis.
• Proficient with data management and information systems.

Responsibilities

• Provide crisis intervention to address immediate needs of the veteran population.
• Assess strengths and needs to develop housing goals.
• Design and carry out a Housing Stability Plan.
• Monitor progress and document outcomes meticulously.
• Ensure compliance with government and contract regulations.
• Facilitate access to services and community resources.
• Conduct home visits and determine eligibility.
