Job summary

Vincentian CARES in Tampa seeks a Case Manager III to provide assessment, planning, linkage and advocacy for veterans in the Supportive Services for Veteran Families program. You will manage a caseload, coordinate housing goals and monitor progress to ensure appropriate services are delivered.

Requires a bachelor’s degree in social work or related field and at least two years of experience with homeless or at‑risk populations.

Qualifications

• Bachelor's degree in social work or related field required.
• Minimum two years’ experience serving homeless or at‑risk populations.
• Proficient with data management and basic Excel, PowerPoint, and Outlook.
• Valid Florida driver’s license and reliable transportation.

Responsibilities

• Provide crisis intervention for very low‑income veterans and families.
• Assess strengths, needs and goals to develop housing plans.
• Design and monitor Housing Stability Plans for each household.
• Maintain progress documentation and comply with regulations.
• Facilitate move to transitional or permanent housing when appropriate.
• Contribute to PQI and accreditation processes.
