Job summary

MidPen Housing Corporation is seeking a Case Manager II in Santa Cruz to support residents in supportive housing. The role involves delivering onsite programming, outreach, crisis intervention, and collaboration with property management to promote stability and self-sufficiency.

Reporting to the Resident Services Manager, you will coordinate with residents, partners, and staff to meet individual goals and ensure trauma-informed care for high-need populations.

Qualifications

• MSW or MA degree in psychology, counseling, or related field with 2 years of full-time experience
• Experience working with individuals who have experienced homelessness or trauma
• Experience using multiple assessment tools and interpreting information from assessments
• Experience collaborating with local service providers and coordinating programs

Responsibilities

• Deliver onsite programming and direct case management services to residents at high-complexity supportive housing sites
• Conduct outreach through door-to-door, phone, and email to engage residents in available services
• Provide crisis intervention and follow-up, including mandated reporting for child or adult abuse
• Facilitate individual and group services that promote independent living, wellness, and recovery
• Collaborate with residents to set and achieve personal and professional goals
• Manage and evaluate services provided by third-party partners and community agencies
• Partner with Property Management and Resident Services teams to address resident concerns
