Job summary

Community Forward SF is hiring a Case Manager II to support women living with HIV in an 18-month transitional housing program. The role also provides case management and housing stabilization for PSH residents at Eddy Street Apartments in Tenderloin, San Francisco.

You will conduct assessments, develop service plans, coordinate services, and advocate for resident-centered goals to promote health and housing stability.

Qualifications

• Bachelor’s degree or equivalent combination of education and experience.
• At least two years of case management experience with adults with complex needs.
• Knowledge of behavioral health, HIV, housing instability, and trauma-informed care.

Responsibilities

• Manage a primary caseload of women living with HIV in transitional housing.
• Develop ISPs aligned to resident goals and housing stability.
• Coordinate with health, behavioral health, housing, and community providers.
• Support residents with benefits, income, and health insurance access.
• Respond to crises with de-escalation and safety protocols.
