Responsibilities

• Conduct comprehensive intakes, eligibility determinations, and barrier assessments.
• Develop individualized stabilization or housing plans with measurable goals.
• Provide trauma-informed coaching, crisis intervention, and problem-solving support.
• Coordinate with internal and external partners for warm handoffs.
• Maintain timely documentation and ensure data quality.
• Respond to crises and conduct safety assessments when needed.
