Job summary

Bill Wilson Center in Santa Cruz, CA is seeking a Case Manager to provide intensive, goal‑oriented support to housing‑stability participants using local systems. You will work under the Program Manager to help clients achieve education, employment and independent living goals.

The role requires a Bachelor's degree in Social Work or related field, 2+ years of case management (3+ years field experience preferred), strong communication, bilingual English/Spanish preferred, and willingness to

Qualifications

• Bachelor's degree in Social Work, Sociology, Psychology or related field.
• 2+ years of relevant case management experience; 3+ years in the field preferred.
• Bilingual English/Spanish is highly desired.
• Must be organized, able to communicate under pressure, and maintain confidentiality.

Responsibilities

• Conduct comprehensive intake and assessment to determine eligibility and needs.
• Develop housing stability plans focusing on education, employment and independent living.
• Provide person-centered support to overcome barriers such as DV, substance use, or mental health challenges.
• Conduct weekly/bi-weekly meetings to monitor progress toward self-sufficiency.
• Coordinate with Coordinated Entry for referrals to housing opportunities.
• Maintain real-time data entry into AWARDS and HMIS.
