Housing Works is seeking a Housing Case Manager to deliver comprehensive case management and care coordination to residents living with HIV or co-occurring conditions. The role involves home visits, documentation, and collaboration with multiple service partners to promote housing stability.

You will support clients with assessments, service plans, and referrals while navigating housing transitions and financial management. The position requires strong advocacy and adherence to program standards.

Formación

• Minimum of three years of relevant experience in social services with individuals who have experienced homelessness, mental health issues, and/or substance use.
• Bachelor's degree in social work, psychology, counseling, or a related field preferred.

Responsabilidades

• Provide case management services and care coordination to a caseload of residents living with HIV and/or co-occurring conditions, including home visits and assessments.
• Document all services and interventions per funder, regulatory, industry and internal standards.
• Coordinate with clients and other departments to connect services and support housing stability.
• Assist clients with moving into apartments and transitioning from homelessness to stable housing.
