Job summary

Community Forward SF is hiring a Case Manager II for the Coronado Hotel, a permanent supportive housing site in San Francisco. You will partner with residents to support housing stability, health, safety, and self-determination, providing information, education, and referrals.

Responsibilities include managing caseloads, developing individualized case plans, facilitating groups, and coordinating with city databases.

Qualifications

• Bachelor's degree or equivalent in education and work experience.
• Case management experience or training.
• Experience with homelessness, trauma, housing instability.
• Strong written and oral communication.
• Proficient in MS Office, Google Suite, ONE and Epic.
• Knowledge of San Francisco Bay Area resources.
• Trauma-informed, person-centered care principles.
• Calm and compassionate under stress.
• Ability to work independently and in a team.
• TB clearance and CPR/First Aid within 90 days.

Responsibilities

• Establish and maintain a caseload of residents and provide services including emotional support, crisis de-escalation, assessment, evaluation, self-advocacy support, rental assistance, and co-facilitation of groups.
• Meet with residents regularly to develop and execute a case plan centered on their goals and timelines.
• Ensure access to resources that stabilize residents' income through benefits advocacy, housing, medical status, and other needs.
• Write clear, objective, and strength-based case notes and complete required documentation timely.
• Maintain resident files and documentation with privacy and accuracy.
• Plan, facilitate, or co-facilitate one resident support, skill-building, or community group per week.
• Assist residents in developing external support networks and community connections.
• Maintain professional boundaries and safety protocols while delivering care rooted in respect.
• Ensure residents are treated with dignity and respect at all times.
• Support mission to promote stabilization, hope, and healing for adults experiencing homelessness.
• Practice self-awareness and proactive stress-management strategies.
• Training and meeting attendance.
• Other duties as assigned.
