Job summary

Housing Works in New York City is seeking a Housing Case Manager to provide comprehensive case management and care coordination for residents living with HIV and co-occurring conditions. You will work in clients’ homes, in the office, and in the community to promote housing stability, independent living, financial management, and health and wellness.

Qualifications

• Bachelor's degree in social work, psychology, counseling, or a related field preferred; Master’s degree a plus.
• Minimum of three years of relevant experience in a social services setting working with homelessness, mental health issues, and/or substance use.

Responsibilities

• Provide case management services to assigned clients, including supportive counseling, motivational interviewing, crisis intervention, care coordination, linkage and referral, assessment and service plan development, field work, and monthly in-home assessments.
• Documentation of all services and interventions provided per funder, regulatory, industry and internal standards.
• Provide referrals and follow-up for primary care, substance use services, nutrition, clothing, mental health services, and financial management.
• Maintain meaningful engagement with residents using motivational interviewing and other interventions to promote independence.
• Coordinate with internal and external departments to connect residents with services.
• Assist clients with moving into apartments and transitioning from homelessness to stable housing.
• Develop strength-based individualized goals with clients and provide feedback on progress.
