Job summary

Community Forward SF is hiring a Case Manager II in San Francisco to support women living with HIV within an approximately 18-month transitional housing program and to provide case management and housing stabilization for residents in Eddy Street Apartments.

You will identify needs, develop service plans with residents, coordinate with health and housing partners, and advocate for access to benefits and resources to promote health, safety, and housing stability.

Qualifications

• Bachelor’s degree or equivalent combination of education and relevant work experience.
• At least two years of case management experience supporting adults with complex needs including HIV.
• Strong knowledge of behavioral health and housing stability.
• Experience coordinating with healthcare, behavioral health, housing, benefits, and community-based providers.
• Knowledge of confidentiality requirements for health information, including HIV-related data.

Responsibilities

• Establish and maintain a primary caseload of women living with HIV in transitional housing and provide case management and housing stabilization support to PSH residents.
• Build trauma-informed relationships with residents using therapeutic engagement strategies to support goals.
• Conduct intakes and assessments to identify strengths, needs, priorities, and resources and orient new residents to housing programs.
• Develop, implement, and update Individual Service Plans based on assessments and resident goals.
• Support residents in identifying and pursuing goals related to health care, benefits, income, and housing readiness.
• Coordinate with outside health, behavioral health, housing, and benefits providers for continuity of care.
• Maintain resident files and ensure privacy protections and documentation compliance.
• Participate in supervision, trainings, and team meetings; maintain professional boundaries.
