Job summary

Housing Works seeks a Housing Case Manager to provide comprehensive case management and care coordination for residents living with HIV and/or co-occurring conditions. You will work in clients' homes, in the office, and in the community to promote housing stability, independent living, financial management, and health and wellness.

Qualifications

• Minimum of three years of relevant experience in a social services setting working with individuals who have experienced homelessness, mental health issues, and/or substance use.
• Bachelor's degree in social work, psychology, counseling, or a related field preferred.

Responsibilities

• Provide case management services including counseling, care coordination, linkage, and service plan development.
• Document all services and interventions per funder, regulatory, industry and internal standards.
• Successfully complete monthly home visits for 100% of clients and ensure HIPAA forms and income verification are uploaded.
• Collaborate with clients to develop initial assessments and service plans within 30 days of enrollment and reassess every 90 days.
• Provide referrals and follow-up for primary care, substance use services, nutrition, mental health, and financial management.
• Promote resident engagement and coordinate with internal/external departments to connect with needed services.
• Assist clients with moving into apartments and transitioning from homelessness to stable housing.
