Qualifications

• 2+ years in community-based health care or social work, or 1 year as Case Manager I.

• Access to reliable transportation with current license and insurance.
• Commitment to serving low-income diverse populations.
• Familiarity with Enhanced Care Management (ECM) and care coordination is a plus.

Responsibilities

• Outreach to patients at LifeLong sites and community settings.
• Engage patients to assess strengths and needs.
• Design and deliver patient-centered care plans aligned with goals.
• Provide and facilitate referrals and assist with applications.
• Maintain a patient caseload per LifeLong standards.
• Use data registries to manage caseload and meet grant deliverables.
• Provide health education and harm reduction information to patients.
• Assist patients with public benefits and housing.
• Communicate with care teams and partners regularly.
• Document patient contacts in required data systems (EHR, HMIS).
• Promote diversity, equity, inclusion, and belonging.
