The Salvation Army USA Western Territory Harbor Light Center is seeking a Recovery Case Manager to provide individualized case management for residents with substance abuse disabilities, focusing on housing and reentry planning.

Under licensed supervision, you will conduct assessments, develop treatment plans, coordinate services, run group sessions, and document progress and discharge summaries. This is a full-time, non-exempt role with competitive benefits.

Qualifications

• Bachelor's degree preferred; five years' experience may be accepted.
• CAT preferred.
• Minimum of 1 year experience in providing case management to high needs populations.
• Minimum of 1 year in addiction counseling preferred.
• Non-profit setting experience preferred.
• Experience with homelessness, veterans, economically disadvantaged, formerly incarcerated, or culturally diverse populations.

Responsabilités

• Provide direct program services to Harbor Light residents using CBT and strength-based approaches, including case management, crisis intervention, treatment plans, relapse prevention, and housing plans.
• Conduct comprehensive screenings and assessments to collect functional, environmental, psychosocial, financial, employment, housing, education and health information.
• Assess barriers and develop case plans; coordinate planning with clients to meet needs.
• Provide service linkages and support systems to meet identified needs; coordinate case conferences and monitor progress.
• Run weekly group therapy and educational sessions; document treatment and discharge summaries.
• Maintain assessments and outcomes pertaining to Pathway of Hope and VA housing contracts; ensure program rules are followed.
