Job summary

Children’s Rescue Fund in New York, NY seeks a Full-time Case Manager to provide direct services including assessment, counseling, crisis intervention, referrals, and advocacy to help families secure permanent housing.

You will work at the Central Park West location, coordinating intake, service plans, and housing steps while maintaining DHS CARES documentation and ensuring compliance with city regulations.

Qualifications

• Bachelor’s degree is preferred, and at least an Associate’s degree in Social Work, Sociology, Psychology, Counseling, Criminal Justice, or Human Services is required
• At least one year of experience in social services, counseling, crisis intervention, family services, preventive services, or housing services
• Experience facilitating groups for homeless adults, knowledge of substance abuse issues and treatment, and/or family dynamics treatment experience
• Bilingual language ability is preferred
• Excellent written and verbal communication skills

Responsibilities

• Complete family intake assessments, service plans/independent living plans, housing plans, next step conferences/transfers, incident reports, progress notes using CARES
• Review information from DHS/HRA/ACS/Probation/ DHS etc. using CARES
• Update all information in CARES in a timely fashion per DHS requirements
• Provide advocacy and referral services to secure benefits and services for permanent housing
• Coordinate services to stabilize the family unit and address substance abuse, housekeeping, parenting, and financial management skills
• Assess clients' needs, create plans, monitor progress, and document interactions to ensure compliant service delivery
• Develop helpful relationships to support residency and involvement in achieving goals
• Provide individual counseling and crisis intervention to stabilize families
• Familiarize with off-site and on-site resources and refer to beneficial services
• Facilitate groups addressing family issues and life skills for self-sufficiency
• Coordinate services with other staff and ensure timely delivery and regulatory compliance
• Communicate effectively among staff and ensure smooth operations and accurate client record transfers
• Perform other duties including on-call rotations and client transfers when required
