Job summary

Children’s Rescue Fund seeks a direct case management professional to provide assessment, counseling, crisis intervention, referral, and advocacy. The role supports relocation of families to permanent housing from the Lighthouse location in Long Island City, Queens.

The incumbent will handle intake assessments, service and housing plans, and progress notes using CARES, and coordinate with DHS, HRA, ACS, and Probation to ensure timely, compliant care.

Qualifications

• Bachelor’s degree or Associate’s degree in Social Work, Sociology, Psychology, Counseling, Criminal Justice, or Human Services.
• At least 1 year of experience in social services, counseling, crisis intervention, family services, preventive services, or housing services.
• Experience facilitating groups for homeless adults and knowledge of substance abuse/family dynamics treatment.
• Bilingual language ability preferred; strong written and verbal communication skills.

Responsibilities

• Administer family intake assessments, service plans, housing plans, and progress notes using CARES.
• Review information from DHS, HRA, ACS, Probation, and DHS CARES data sources.
• Update all client information in CARES in a timely and compliant manner.
• Provide advocacy and referrals to help families access benefits and services for permanent housing.
• Coordinate services to stabilize families and address substance use, housekeeping, parenting, and financial skills.
• Assess client needs, develop service plans, monitor progress, and document interactions to ensure effective service delivery and compliance.
• Build supportive relationships with clients to facilitate residency at the facility and active involvement in goals.
• Provide individual counseling and crisis intervention to stabilize families during displacement.
• Refer families to off-site and on-site resources to maximize benefits.
• Facilitate group sessions addressing family issues and life-skills development.
• Coordinate client services with other staff and ensure timely regulatory compliance.
• Ensure effective communication among staff and proper transfer of information for admissions/discharges.
• Perform other duties and special projects, including quarterly on-call rotations.
• Assist in moving clients in and out of the program when required.
