Job summary

Children’s Rescue Fund in Long Island City, NY is seeking a Case Manager (Lighthouse) to provide direct case management, including assessment, counseling, crisis intervention, referral, and advocacy for families to secure permanent housing.

The role requires coordination with DHS CARES and partner agencies, thorough documentation of services, and support for families facing housing instability. Strong communication and organizational skills are essential.

Qualifications

• Bachelor’s degree preferred; at least an Associate’s degree in Social Work, Sociology, Psychology, Counseling, Criminal Justice, or Human Services required
• At least one year of experience in social services, counseling, crisis intervention, family services, preventive services, or housing services
• Experience facilitating groups for homeless adults; knowledge of substance abuse issues and family dynamics treatment preferred
• Bilingual language ability preferred
• Excellent written and verbal communication skills

Responsibilities

• Complete intake assessments, service plans/independent living plans, housing plans, transfers, incident reports, and progress notes using CARES
• Review information from DHS, HRA, ACS, Probation, DHS and other agencies via CARES
• Ensure information is updated in the system in a timely fashion
• Provide advocacy and referral services to secure benefits and services for a transition to permanent housing
• Coordinate services to stabilize families and improve skills for permanent housing
