Job summary

Children's Rescue Fund seeks a dedicated case manager to provide direct services including assessment, counseling, crisis intervention, and advocacy to support families toward permanent housing. You will use DHS CARES to manage intake, service plans, and progress notes, while coordinating with partner agencies for seamless service delivery.

The role requires strong communication, group facilitation skills, and the ability to work within regulatory requirements.

Qualifications

• Bachelor’s degree preferred.
• Associate degree in Social Work, Sociology, Psychology, Counseling, Criminal Justice or Human Services.

Responsibilities

• Complete intake assessments and service plans using DHS CARES system.
• Provide advocacy and referrals to access benefits and housing services.
• Coordinate services to stabilize families and assist with housing transitions.
• Document interactions and progress notes in accordance with regulations.
• Assist with on-site and off-site program resources and group facilitation.
