PAM Health Rehabilitation Hospital of Palo Alto is seeking a Case Manager to coordinate interdisciplinary care for a defined patient population. You will review admission records to ensure appropriate resource use and admission levels, assess discharge needs, and coordinate discharge planning.

The role involves working with the interdisciplinary team to achieve desired financial and quality outcomes and to maintain payor relationships.

Competenze

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker.
• Current BLS certification required.
