PAM Health is seeking a Case Manager to coordinate interdisciplinary care for a designated patient population within our hospital.

You will review inpatient admission records to ensure proper utilization of resources, assess discharge needs, and coordinate discharge planning while collaborating with the care team to achieve financial and quality outcomes.

Licensure in the state is preferred; 3–5 years of inpatient experience (acute, IRF, LTACH) and current BLS certification are required.

Formación

• Education and Training: Licensure in the state where the hospital resides is preferred.
• Professional disciplines of the designated case manager may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker.
• Current BLS certification required.

Responsabilidades

• Coordinate interdisciplinary care for a designated patient population.
• Review inpatient admission records to ensure proper utilization of hospital resources and determination of admission for appropriate level of care.
• Assess and identify discharge needs and coordinate appropriate discharge plan.
• Work collaboratively with the interdisciplinary team to facilitate achievement of desired financial and quality outcomes.
• Maintain and enhance payor relationships.
