Job summary

PAM Health in the United States is seeking a Case Manager to coordinate care across inpatient settings. You will review admission records, assess discharge needs, and work with the interdisciplinary team to ensure appropriate level of care and efficient resource use.

You will also maintain and strengthen payor relationships while supporting quality outcomes and patient-centered planning in a dynamic hospital environment.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, RN, respiratory therapist, speech therapist, or social worker.
• Current BLS certification required.

Responsibilities

• Coordinate care for a designated patient population.
• Review inpatient admission records for proper utilization of hospital resources and admission level of care.
• Assess discharge needs and coordinate discharge plan.
• Collaborate with the interdisciplinary team to facilitate quality outcomes.
• Maintain and enhance payor relationships.
