Job summary

PAM Health Warm Springs Rehabilitation Hospital of Northeast San Antonio is seeking a Case Manager to coordinate care for a designated patient population. You will review inpatient admission records to ensure appropriate utilization of hospital resources and determine admission levels of care, while planning and coordinating discharge needs.

You will collaborate with an interdisciplinary team to achieve financial and quality outcomes and maintain payor relationships.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
• Current BLS certification required.
• Three to five years of inpatient experience in acute, IRF, or LTACH setting preferred.

Responsibilities

• Coordinate care for a designated patient population across inpatient admission records to ensure proper utilization of hospital resources and determine admission level of care.
• Assess and identify discharge needs and coordinate appropriate discharge plan.
• Work with the interdisciplinary team to facilitate achievement of financial and quality outcomes and maintain Payor relationships.
