Job summary

PAM Health is seeking a Case Manager to oversee interdisciplinary coordination of care for a designated patient population. You will review admission records to ensure appropriate utilization of hospital resources and determine admission levels of care.

You will assess discharge needs, coordinate the discharge plan, and work with the team to optimize financial and quality outcomes while maintaining payor relationships. Licensure in Texas and a current BLS certification are required.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Current BLS certification required.
• Three to five years of inpatient experience in acute, IRF, or LTACH settings.

Responsibilities

• Coordinate interdisciplinary care for designated patient populations.
• Review inpatient admission records to ensure appropriate level of care.
• Assess discharge needs and coordinate the discharge plan.
• Collaborate with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
