Qualifications

• Current BLS certification required.
• Licensure in the state where the hospital resides is preferred.

Responsibilities

• Coordinate care for designated patient populations within the interdisciplinary team.
• Review inpatient admission records to ensure proper utilization of hospital resources and determine admission level of care.
• Assess and identify discharge needs and coordinate the discharge plan.
• Collaborate with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
