Job summary

PAM Health is seeking a Case Manager to coordinate the interdisciplinary care for a designated patient population and to review inpatient records for appropriate utilization. The role includes assessing discharge needs and coordinating discharge planning, while working to enhance payor relationships and collaborate with the care team.

Ideal candidates will have 3–5 years of inpatient experience and state licensure with current BLS certification, supporting high-quality outcomes in a hospital

Qualifications

• Licensed professional in the relevant field; licensure in the state preferred.
• Current BLS certification is required.

Responsibilities

• Coordinate care for designated patient population through interdisciplinary collaboration.
• Review inpatient admission records to ensure appropriate level of care and utilization.
• Assess discharge needs and coordinate discharge planning.
• Maintain and enhance payor relationships.
• Collaborate with the interdisciplinary team to achieve desired financial and quality outcomes.
