Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Case Manager to coordinate interdisciplinary care for a designated patient population. You will review inpatient records to ensure proper utilization and determine admission levels, while identifying discharge needs and planning coordination of discharge plans.

In this role, you will work with the interdisciplinary team to achieve desired financial and quality outcomes and maintain/payor relationships.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Designated case manager roles may include OT/PT/RN/RRT/SLP or licensed social worker.
• Current BLS certification is required.
• 3–5 years inpatient experience, preferably in acute, IRF or LTACH.

Responsibilities

• Coordinate care across disciplines for a designated patient population.
• Review inpatient admission records to ensure appropriate level of care.
• Assess discharge needs and coordinate the discharge plan.
• Collaborate with the team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
