Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, RN, respiratory therapist, SLP, or social worker.

Responsibilities

• Coordinate care across interdisciplinary team for designated patient populations.
• Review inpatient admission records for appropriate level of care and resource utilization.
• Assess discharge needs and coordinate discharge planning.
• Maintain and enhance payor relationships.
