Job summary

VOALA is seeking a Case Manager Generalist to coordinate services for clients with low acuity. You will conduct screenings, develop case plans, and connect clients to needed resources, serving as the primary point of contact to ensure coordinated care.

This on-site role requires fingerprint clearance and the ability to work with diverse populations. A HS/GED and/or a bachelor’s degree in social services, plus 1 year of related experience, are preferred.

Qualifications

• Fingerprint clearance and background checks required.
• Ability to work under emotional stress with vulnerable clients.
• Education: HS/GED; VOALA Case Management training within 6 months of hire.
• Experience: Minimum 1 year in case management or a related social services role; a Bachelor’s in a related field can substitute.
• Preferred: Bachelor’s Degree in Social Work/related field and HMIS navigation.

Responsibilities

• Conduct screenings and assessments to collect functional, environmental, psycho-social, financial, employment, housing, educational and health information.
• Coordinate case conferences, monitor progress, and advocate for clients with agencies and community partners.
• Develop and implement individualized case plans and connect clients to necessary services.
• Perform on-site client services and maintain detailed documentation.
