Job summary

VOALA, a VOALA program, seeks a Case Manager (Specialist) to provide intensive case management to eligible clients, meeting weekly. You will be the primary contact to coordinate services and care for assigned clients, handling a caseload across diverse human services.

The role requires on-site work, fingerprint clearance, and the ability to support clients facing housing, health, and social challenges in a dynamic environment.

Qualifications

• Minimum 1 year general experience in case management or similar social services role.
• Fingerprint clearance/background check required.
• Ability to work with potentially challenging clients and high-stress situations.

Responsibilities

• Conducts comprehensive screenings and assessments to develop a case plan.
• Provide service linkages and support systems to meet identified needs.
• Coordinates individualized planning with clients to meet short- and long-term needs.
• Advocates for clients before agencies and coordinates care with service teams.
